New annual health plan fee begins next month

Employers who offer health plans could feel the impact of a new annual fee as soon as next month. Depending on when an employer’s health plan ends, the first annual fee to support the Patient Centered Outcomes Research Institute (PCORI) is due July 31.

PCORI was created under the health care law to research evidence-based health care and outcomes. The annual fees, from which PCORI receives part of its funding, will be in place for the next seven years. Insurers and plan sponsors will pay one dollar per covered individual under a health plan in the first year, two dollars per individual in the second year, and indexed amounts in years three through seven.

Health insurers will pay the fee for fully insured plans, but the costs likely will be passed on to employers. The sponsors of self-insured plans—likely employers—also will pay the fee. Those who are required to pay the PCORI fee must file IRS Form 720 to report and pay.
