How to Complete the Survey

You can complete the survey in one of two ways:

• Complete an electronic version of the survey and submit it by e-mail:

1. Download the survey as an Excel file (965 KB).
2. Complete the survey on your computer by entering your answers in the Excel file.
Save the file.
3. E-mail the file to 2008NRA@peoplereport.com.

OR

• Complete a hard copy version of the survey and submit it by mail:

1. Download the survey as a PDF file (416 KB).
2. Complete the survey.
3. Mail the completed survey to:

Please submit the completed survey by Dec. 19, 2008. The National Restaurant Association will provide a FREE copy of this valuable report to all participants who return a completed questionnaire by Dec. 19, 2008.
