You also can call (800) 424-5156, ext. 3665, to request a copy by mail. The survey is for owners/operators/managers. If you own a single restaurant, complete one survey. If you own two or more restaurants, complete surveys for each unit you operate or for as many units as possible. The two-part survey asks for (1) general information on your restaurant, and (2) selected income and expense data. If you feel you can't complete the financial portion, you can either have your accountant supply the data or send Deloitte your latest 12-month detailed financial statements and Deloitte will complete this part of the survey for you. All information is kept strictly confidential. We detach your return address from the survey as soon as we receive it; data is used only for compiling industry totals.
